It is worth being honest about the Outer Sunset before recommending anything. The far end of the avenues is one of the most interesting eating neighbourhoods in San Francisco and almost none of it is Mediterranean. What is out here is Chinese, Vietnamese, Korean, Thai and Burmese, plus a layer of surf-town cafés and bakeries. Searching for Mediterranean food near you at 45th and Noriega and getting a thin, slightly desperate list is not a failure of the search — it is an accurate reflection of the neighbourhood.
The good version of this cuisine is a fixed distance east rather than a hidden door nearby, and the ride is short. Tell Karpo where you are on the avenues and it will tell you how far you actually have to go.
Where it actually is
The Sunset's Mediterranean and Middle Eastern cooking sits on Irving Street in the Inner Sunset, along the commercial stretch that runs from around 5th Avenue to 12th Avenue and up 9th Avenue between Lincoln Way and Judah Street. That is the neighbourhood's dense restaurant strip, and it is where the kebabs are.
Yumma's on Irving Street does shish kebabs of marinated chicken and ground beef, alongside hummus, falafel, saffron rice and a Greek-style salad. It is the straightforward answer to the question — a kebab house doing kebab-house things well, in a neighbourhood that mostly is not doing them at all.
The one that got there first
Sunrise Deli at 2115 Irving Street opened in 1984 and is credited with introducing Middle Eastern food to a great deal of the Bay Area. Forty-plus years in one location is the kind of longevity that tells you more than any review does, and a deli format means you can eat there or carry it away, which matters if the actual plan is to take it back toward the beach.
Between Yumma's and Sunrise Deli you have the two registers — sit down with a plate of grilled meat, or take a wrap away — and that is genuinely most of what this cuisine offers in the Sunset. It is a short list, and a short list of good places is more useful than a long list padded out with places that happen to sell a wrap.
What the far avenues do instead
If the drive east is not happening, the honest substitution is to eat what the neighbourhood is actually good at. Old Mandarin Islamic Restaurant at 3132 Vicente Street is entirely halal Northern Chinese, and its cumin-heavy stir-fried lamb, lamb ribs and lamb dumplings occupy roughly the same flavour territory as a kebab — grilled lamb, cumin, chilli, bread — arrived at from a completely different direction. For anyone who wanted Mediterranean because they wanted grilled lamb, this is a better meal than a mediocre wrap.
Palm City at 4055 Irving Street, out at the far end, does footlong Italian-American hoagies with a proper wine list — Mediterranean in the broadest sense, and the best room out here for sitting down with something cured and a glass of red.
How far it actually is
The N Judah light rail runs the length of Judah Street from Ocean Beach through to the Inner Sunset and beyond, which makes the Irving Street strip a straightforward ride rather than an expedition — you are crossing about twenty blocks, not the city. The 7 covers Noriega and the 71-area routes serve Irving. Driving is easy in both directions and parking is far harder at the Inner Sunset end than in the avenues, which is the one argument for taking the train.
Choosing
You want kebabs and you are willing to travel: Yumma's on Irving. You want a wrap to take away: Sunrise Deli. You want grilled lamb and cumin without leaving the far avenues: Old Mandarin on Vicente. You want cured meat, bread and wine, walking distance from the beach: Palm City on Irving at 40th.

Note that Irving Street runs the full width of the district, so a 2000-block Irving address and a 4000-block Irving address are about twenty blocks apart — checking the number rather than the street name is the thing that prevents a wasted trip out here.
